423 Up Against the Wall - Emergency Double Myocardial Rupture Repair

چکیده
Abstract Background Left ventricular free wall rupture (LVFWR) and interventricular septal (VSR) are potentially catastrophic mechanical complications after acute myocardial infarction (AMI). When they occur together ("double rupture" (DMR)), survival is unlikely. DMR seen in only 0.3% of all AMIs. With or without surgical intervention, the odds against patient. Case presentation Our 57-year-old male patient self-referred to emergency department a remote hospital 5 days first experiencing chest pain. Investigations ED confirmed an inferior ST-segment elevation (STEMI) complicated by DMR. Coronary angiography revealed mid-course total occlusion right coronary artery (RCA). He was rapidly transferred our regional cardiac unit, arriving at theatre cardiogenic shock but conscious before arresting being massaged onto bypass. Not did he survive all-night operation, requiring mitral valve replacement process, survived multiple ITU be eventually transferred, neurologically intact, on postoperative day 66 peripheral unit complete his rehabilitation. has been discharged from rehabilitation centre 88 now able ambulate with walking frame. Conclusions patient, odds, complications. We present details case literature surrounding condition. hope patients meagre probabilities, such as this case, not given up on; hence, final message report persevere persist odds.

متن کاملVentricular free wall rupture after myocardial infarction
Patients with their first myocardial infarction (MI), who present to the emergency department many hours after the onset of chest pain, who appear to be improving but suddenly develop new chest pain and unexpected hypotension (with or without signs of cardiac tamponade), should be suspected of having ventricular free wall rupture (VFWR). The mainstay of treatment is surgery. These patients may ...
متن کاملrepair of post-infarction ventricular free wall rupture with tachosil®
introduction left ventricular free wall rupture (lvfwr) is a frequent cause of death after acute myocardial infarction, and its repair remains a surgical challenge. conclusions a hemodynamic study was acceptable at a 12-month follow-up, and no complication was seen. case presentation tachosil® is a ready-to-use equine collagen patch which has been successfully used for hemostasis in cardiovascu...
متن کاملRepair of ventricle free wall rupture after acute myocardial infarction: a case report
INTRODUCTION Acute myocardial infarction (AMI) may culminate in sudden death by ventricular fibrillation, cardiogenic shock, and cardiac rupture. We present a case of postinfarction rupture treated by direct closure and coronary artery bypass grafting after thrombolytic therapy. CASE REPORT A 67-year-old woman with cardiac risk factors of hypertension, diabetes mellitus, and being post-menopa...
